RESUMO

OBJECTIVE: To observe the intervention effect of damage control orthopaedic(DCO) strategy on fat embolism syndrome(FES) associated with long shaft fracture of lower limbs. METHODS: Retrospective analysis was made on the clinical data of 163 patients with FES associated with lower limb long shaft fractures admitted from January 2015 to May 2021. They were divided into two groups based on the time point of implementing DCO strategy in January 2018. Total of 92 patients were admitted from January 2015 to December 2017 as the control group, and other 71 patients were admitted from January 2018 to May 2021 as the intervention group. The hospital mortality, arterial oxygen saturation (SaO2), arterial partial pressure of oxygen (PaO2) and oxygenation index (OI), hemoglobin (Hb), platelet count(PLT), Harris score of hip joint, HSS score of knee joint, AOFAS score of ankle joint, clinical efficacy and complications were observed and compared between two groups. RESULTS: Total of 163 patients were followed up for 12 to 18 months with an average of (16.91±1.22) months. The in-hospital mortality rate in the intervention group was 2.82% (2/71), and that in the control group was 16.30% (15/92), the difference between two groups was statistically significant(χ2=6.455, P<0.05). After the intervention, SaO2, PaO2 and OI in two groups were higher than those before the intervention(P<0.05), and after the intervention, SaO2, PaO2 and OI in two groups were statistically significant(P<0.05). Hb and PLT in two groups after intervention were higher than those before intervention (P<0.001), and there was statistically significant difference in Hb and PLT between two groups after intervention (P<0.05). The Harris score of hip joint, HSS score of knee joint and AOFAS score of ankle joint in both groups after 3 months of treatment were better than those before treatment (P<0.05). The total clinical effective rate of the intervention group was higher than that of the control group(χ2=4.194, P<0.05). The total incidence of complications in the intervention group was lower than that in the control group(χ2=4.747, P<0.05). CONCLUSION: DCO strategy is helpful to reduce the in-hospital mortality of patients with FES associated with long shaft fracture of lower extremities, eliminate FES symptoms and stabilize vital signs, gain time advantage for phase Ⅱ definitive surgery, and has significant clinical intervention effect, which is worth popularizing.

BACKGROUND: Lumbar disc herniation is a common disease. Endoscopic treatment may have more advantages than traditional surgery. AIM: To compare the clinical efficacy and safety of microendoscopic discectomy (MED) and open discectomy with lamina nucleus enucleation in the treatment of single-segment lumbar intervertebral disc herniation. METHODS: Ninety-six patients who were operated at our hospital were selected for this study. Patients with single-segment lumbar disc herniation were admitted to the hospital from March 2018 to March 2019 and were randomly divided into the observation group and the control group with 48 cases in each group. The former group underwent lumbar discectomy and the latter underwent laparotomy and nucleus pulpectomy. Surgical effects were compared between the two groups. RESULTS: In terms of surgical indicators, the observation group had a longer operation time, shorter postoperative bedtime and hospital stay, less intraoperative blood loss, and smaller incision length than the control group (P < 0.05). The excellent recovery rate did not differ significantly between the observation group (93.75%) and the control group (91.67%). Visual analogue scale pain scores were significantly lower in the observation group than in the control group at 1 d, 3 d, 1 mo, and 6 mo after surgery (P < 0.05). The incidence of complications was significantly lower in the observation group than in the control group (6.25% vs 22.92%, P < 0.05). CONCLUSION: Both MED and open discectomy can effectively improve single-segment lumbar disc herniation, but MED is associated with less trauma, less bleeding, and a lower incidence of complications.

RESUMO

Two new flavonoid glycosides, named viscumneoside XII (1), and viscumneoside XIII (2); a new dihydrogen flavonoid glycoside product named viscumneoside XIV (3), were isolated from the aerial part of Viscum album, along with seven known compounds (4-10). Their structures were identified by analysis of spectroscopic data. In addition, cytotoxicity assay showed that 1, 2 and 3 possessed significant inhibitory activities against C6, A549 and MDA-MB-231 (the inhibition rate arrived about 50%, 70% and 74% respectively with IC50 ≤ 60.00 µmol·L-1), while the inhibition of TF-1 and Hela was not significant.

In the natural state, the concentration of carbon dioxide in the atmosphere is about 300 µmol mol-1. Plants need a suitable balance of CO2 to achieve optimal growth. The optimum CO2 content corresponding to a high photosynthesis rate is between 0.1 and 1.0% by volume. However, air has only a CO2 content of 0.03% by volume, so plants cannot use all of their growth potential. The use of fertilizer to assist in the supply of CO2 increases the rate of photosynthesis. In this work, a slow-release CO2 gas fertilizer inspired by polyphenol chemistry was prepared to provide sustainable CO2 that could improve plant photosynthetic capacity and get a higher crop yield. The core-shell structure was designed to confer gas fertilizers slow-release property. Micron-sized calcium carbonate particles with uniform particle size and regularity morphology, as carbon sources for plant photosynthesis, was a core, and tannic acid was coated on it as a shell via oxidative oligomerization and cross-linked by polyetherimide. The structure and morphology of fertilizers were characterized by scanning electron microscopy, X-ray energy dispersive spectroscopy, transmission electron microscopy, X-ray photoelectron spectroscopy, and thermogravimetric analysis. In vitro experiments, the prepared fertilizers were proved to have slow-release properties. And then through net photosynthesis rate, chlorophyll fluorescence parameters, chlorophyll content, leaf area, leaf mass per area, and dry matter to study the effects of slow-release CO2 gas fertilizers on plant physiology of Brassica chinensis. The results revealed that the slow-release CO2 gas fertilizers not only had good slow-release properties but also can well improve plant photosynthesis.

A distinct dichotomy in product distribution was initially observed in the silver-catalyzed reaction of trifluoromethyl (CF3) ketones with terminal alkynes having two different types of electronic natures. The domino reaction smoothly proceeded almost exclusively with high stereoselectivity with terminal alkynes containing ester groups, whereas alkynylation occurred in good yield when terminal alkynes containing aryl or alkyl groups were present. The results indicated that the electronic nature of terminal alkynes can act as a switch that enables either the domino reaction or alkynylation between terminal alkynes and CF3 ketones.
